Terms & Conditions

These Terms and Conditions apply to the supply of all services by The Lymm Eye Clinic. 

 

Definitions

In these terms and conditions the following words and expressions shall have the meanings respectively set against them: 

Doctor“: A general medical practitioner being registered to practice with the General Medical Council; 

Non-Surgical Treatment“: Treatment supplied by The Lymm Eye Clinic not being a surgical procedure; 

Nurse“: A nurse registered to practice by the Nursing and Midwifery Council; 

Patient“: Any person who has entered or is contemplating entering into a contract for a Surgical Procedure or Non-Surgical Treatment from The Lymm Eye Clinic; 

Relevant Practitioner“: A Nurse, Aesthetic Practitioner, Surgeon or Doctor (as the case may be); 

Relevant Treatment“: The Non-Surgical Treatment booked through The Lymm Eye Clinic carried out or to be carried out on the Patient by a Relevant Practitioner; 

The Lymm Eye Clinic”: means The Lymm Eye Clinic, 1. Birchbrook Rd, Lymm. WA13 9RR. 

 

The Lymm Eye Clinic’s Obligations

In providing a Relevant Treatment for a Patient, The Lymm Eye Clinic shall:

Ensure that any Non-Surgical Treatment is carried out by a suitably qualified Practitioner;

Provide suitable facilities for the provision Non-Surgical Treatment;

Provide the Patient with such information that can be reasonably expected to be necessary for the Patient to understand the nature of the Relevant Treatment being offered and the risks and conditions normally attendant on such Relevant Treatment.


 

The Patient’s obligations

The Patient shall be responsible for reading and understanding these Terms & Conditions and information/literature provided prior to consenting to undergo any Relevant Treatment. 

 

Payment

The Patient shall pay a fixed fee for all appointments at the end of the appointment.

The fee is £150 which includes a 30 minute outpatient appointment, any imaging or investigation deemed appropriate (which may include: imaging, visual fields examination, OCT examination of the retina or optic nerve) and any communications required as a result of the appointment.

This payment does not include the costs any future treatments if referred elsewhere.

This payment does not include the cost of any medications that may be purchased on site.

Late payment of outstanding balances will incur administrative costs which will be added to any debt. 

 

Complaints

In the event that the Patient is unhappy with any aspect of the service provided, the complaint should be made as soon as possible to the Clinic Manager. If he/she is unable to resolve the complaint to the reasonable satisfaction of the Patient, The Lymm Eye Clinic’s Complaints Procedure shall apply. 

 

Data protection

The Lymm Eye Clinic processes data relating to Patient’s in connection with the Relevant Consultation/Treatment/Procedure provided to those Patients in accordance with these terms and conditions. 

The Lymm Eye Clinic wishes to disclose data relating to Patients to the Surgeons, Doctors, and Nurses in the course of the Relevant Treatment and The Lymm Eye Clinic wishes to have the consent of the Patient to such disclosure. 

The Patient is deemed to consent to the disclosure of sensitive personal data by The Lymm Eye Clinic to Surgeons, Doctors and Nurses for the purposes of discussing the Patient’s Treatment. 

The Patient acknowledges that The Lymm Eye Clinic is obtaining this consent in order to comply with the provisions of the Data Protection Act 2018. 

 

Patient access to medical records 

In line with current legislation and our clinic policy: 

Patients may request to view their medical records, but a member of the medical team must be present. 

Patients who wish to request a copy of their medical records should do so in writing. A copy of the records will be sent within 28 days of receipt of the request. 

The Lymm Eye Clinic reserves the right to charge an administration fee of up to £50 for providing medical records. 

 

Legal jurisdiction

The service provided by The Lymm Eye Clinic and Relevant Practitioners shall be governed by the laws of England and Wales, whose courts shall have exclusive jurisdiction.
